| 3. Shift Selection |
Question 1 indicates the shift times you desire. The majority of shifts are needed Thursday-Sunday. To qualify for rounds of golf at PGA National Resort, you must serve at least three (3) shifts for your committee. Question 2 indicates your willingness to serve additional volunteer shifts above your committee requirements prior to and after the tournament.

| 4. Standard Volunteer Package |
| The standard volunteer package for 2007 will be $60.00. This package, valued at $300.00, includes a shirt, hat, two golf coupons, and two weekly ticket books for the 2007 Honda Classic. Please respond to the information requested for male or female golf shirt size by placing an “checkmark” in the space provided and circle the appropriate size. Additional shirts at a discounted price will be available at uniform distribution. |
